Chaperones will need to register as dividends with SCPS -- can be done on the SCPS website.  Secondly, chaperones who have not already been previously fingerprinted will need to do so; it's a very quick and easy process.  I'll forward information to you individually.   Thanks, Mr. Crisp

Only download Form #6 if you are chaperoning.

You'll first need to please register as a dividend with SCPS.

Secondly, you'll receive a link to arrange a time for fingerprinting.
